Family therapy, which focuses on the family system, is an essential part of psychiatric care. The general framework of the multidisciplinary diagnostic and therapeutic management of mental illness is the concept of syndromes. These are characterized by their cause, their course, and their manifestations (symptoms). The family must also be considered as a distinct diagnostic entity, with a particular course and its own possible forms of manifestation. In the light of the clinical reality of mental illness, the family, and family therapy, we can introduce three different syndromes, characterized by a specific course and clinical manifestations. In the first, the family is considered as the dominant factor in the cause of the illness. In the second, the family is viewed as the facilitator of the expression of mental disorders. In the third, we consider the family as a key factor in the onset of the illness and in the maintenance of the symptoms. This organization of three basic syndromes of the family leads to a taxonomy of four specific syndromes: the family-symptom syndrome, the family-course syndrome, the family-environment syndrome, and the family-behaviour syndrome. By exploring each of these syndromes and their clinical aspects, one can develop better therapeutic strategies for the family.
